Description
A Brief Overview
Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status. This LPN will travel with the provider, primarily to Ahuja, Streetsboro Health Center and Lake West Health Center.
What You Will Do
- Functions in accordance with the professional standards of the Ohio board of nursing.
- Completes/submits required documentation.
- Delivers effective relationship based nursing care.
- Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes.
- Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations.
- Develops self and contributes to the professional practice of nursing.
- Displays leadership that influences optimal clinical and operational outcomes.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications
Education
- Other Accredited Program Completion of accredited practical nurse program
Knowledge, Skills, & Abilities
- Knowledge of LPN scope of practice according to the Ohio board of nursing. (Required proficiency)
- Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (ie. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices) (Required proficiency)
- Level II Only: 18 CNE’s per year. • 3 CNE’s in clinical specialty • At least 5 CNE’s in Leadership • Other CNE requirements will be agreed upon with manager ( proficiency)
- Level II Only: • 3 Peer Reviews. • Committee Involvement. • 5 criteria with evidence of each. ( proficiency)

- Licensed Practical Nurse (LPN), Ohio and/or Multi State Compact License Upon Hire (Required)
- Basic Life Support (BLS) Upon Hire (Required)
