Infectious Diseases Clinical Pharmacy Specialist – Immunocompromised Patient Population
A Brief Overview
UH Cleveland Medical Center (UH CMC) is seeking a highly motivated Infectious Diseases Clinical Pharmacy Specialist to join our pharmacy team. This position will provide dedicated infectious diseases pharmacy support to immunocompromised patient populations, including patients with hematologic malignancies, bone marrow transplantation, solid organ transplantation, and other conditions requiring complex antimicrobial management.
The Clinical Pharmacy Specialist will serve as a subject matter expert in infectious diseases pharmacotherapy, antimicrobial stewardship, and the management of infections in immunocompromised patients. Working collaboratively with Infectious Diseases physicians, oncology teams, transplant teams, advanced practice providers, nursing, and pharmacy colleagues, this specialist will help optimize antimicrobial therapy, improve patient outcomes, and advance quality initiatives at UH Cleveland Medical Center.
Position Summary
This Infectious Diseases Clinical Pharmacy Specialist will:
· Serve as the infectious diseases pharmacotherapy expert for immunocompromised patient populations, including malignant hematology, blood and marrow transplant, cellular therapy, and solid organ transplant patients.
· Participate in and support inpatient Infectious Diseases consult services caring for immunocompromised patients.
· Conduct prospective antimicrobial stewardship reviews focused on broad-spectrum antibacterial, antiviral, and antifungal therapies.
· Optimize antimicrobial selection, dosing, monitoring, and duration of therapy for complex infectious disease states.
· Collaborate closely with Infectious Diseases physicians and multidisciplinary care teams to develop individualized treatment plans.
· Lead antimicrobial stewardship and quality improvement initiatives targeting immunocompromised patients.
· Participate in transitions of care and outpatient antimicrobial management initiatives, including serving as backup support for the UH Cleveland Medical Center OPAT Program.
· Support clinical teaching, research, and scholarly activities within the Department of Pharmacy.
Key Responsibilities
· Review and interpret microbiology, laboratory, pharmacokinetic, and clinical data related to infectious diseases management.
· Optimize antimicrobial therapy through evidence-based recommendations including escalation, de-escalation, IV-to-oral conversion, therapeutic drug monitoring, and antimicrobial allergy clarification.
· Provide stewardship oversight for high-cost and restricted antimicrobial agents.
· Support management of complex bacterial, fungal, viral, and opportunistic infections in immunocompromised patients.
· Participate in multidisciplinary patient care rounds and case discussions.
· Monitor for and prevent antimicrobial-related adverse drug events and medication errors.
· Develop and implement clinical guidelines, protocols, and best practices related to infectious diseases and antimicrobial stewardship.
· Document clinical interventions and outcomes.
· Serve as a resource and educator for pharmacists, physicians, advanced practice providers, nurses, pharmacy residents, and pharmacy students.
· Participate in departmental committees, quality improvement initiatives, and research activities.
Teaching and Research Opportunities

· Precept PGY1 Pharmacy Residents and PGY2 Infectious Diseases, Critical Care, Solid Organ Transplant, and Oncology Pharmacy Residents.
· Deliver educational programs to pharmacists, physicians, advanced practice providers, nursing staff, residents, and students.
· Participate in clinical research and scholarly publication activities.
· Collaborate with academic partners and contribute to advancing infectious diseases pharmacy practice.

Education
· Doctorate Degree in Pharmacy (Required)
· Completion of an ASHP PGY1 and PGY2 in Infectious Disease Pharmacy (Strongly Preferred)
· 3-5 years of experience in the practice area. (Preferred)
Experience
· Advanced training or experience in infectious diseases pharmacotherapy.
· Experience with antimicrobial stewardship programs preferred.
· Experience caring for oncology, hematology, transplant, or immunocompromised patient populations preferred.
· less than 1 year Experience as a pharmacist with training in drug therapy of diseases including the knowledge and application of clinical pharmacology, pharmacokinetics, drug-drug interactions, and pharmaceutics to patients, training in cost analysis/cost evaluation techniques (Preferred)
Knowledge, Skills, and Abilities
· Strong clinical knowledge of infectious diseases pharmacotherapy and antimicrobial stewardship principles.
· Expertise in therapeutic drug monitoring and antimicrobial optimization.
· Excellent communication and interdisciplinary collaboration skills.
· Ability to analyze complex clinical data and provide evidence-based recommendations.
· Demonstrated commitment to teaching, quality improvement, and advancing patient care.
· Computer systems skills in spreadsheet, database and word processing (Preferred proficiency)
· Must demonstrate highly effective written and oral communication skills (Required proficiency)
